How to add images to document forms

Document forms in Bookem are designed to help you collect structured, reusable information. One of their most powerful features is the ability to add images - like body charts or diagrams - that can be drawn on directly during note - taking.

This is especially helpful for clinical assessments, injury tracking, or progress reports where visual context improves clarity and communication.

Tip: If you need more layout flexibility - such as placing text alongside an image, adding tables, or using variables - consider using a document template instead.

Field types available in document forms

When creating or editing a document form, you can choose from the following field types:

  • Text fields – For short or long written responses

  • Drop-downs – Create a list of options or link to a data list

  • Checkboxes – Allow multiple selections

  • Multiple choice – Limit selection to one option

  • Diagrams – Upload images (e.g. body charts or clinical diagrams) that users can annotate

How to add an image using the diagram field

To add an image to a document form:

  1. Go to Documents > Forms

  2. Click + New form, or open an existing form you'd like to edit

  3. Name your form and click Create

  4. Click + Add field, then choose Diagram

  5. Upload your image (e.g. a body chart, anatomy diagram, or visual aid)

  6. Once added, users can draw directly on the image when completing the form

  7. Click Save & close to save your changes

💡 You can include multiple diagram fields in one form if needed.

Using your form in a document

Once your form includes the images you need, you can use it during note-taking:

  1. Open a new or existing document

  2. Click + Insert, then select Form from the drop-down menu

  3. Choose your saved document form

  4. Fill it in, or click Update to insert it blank

  5. Click Create & close or Save & close to save your document

You will be able to access this form during note-taking in Documents by clicking on the '+ Insert > Form' from the drop down at the top right of the document.
